Please read the following statements carefully, and then indicate your willingness to abide by these agreements. PSI Seminars expects all participants to respect the person and property of others. Participation in PSI Seminars’ Teen Experience hinges on this respect, in support of the goals and objectives of this seminar.

1. Participate in all exercises and activities, arriving at the scheduled place on time.

2. Sleep only in my assigned bed, in my assigned sleeping area, being respectful of my roommates’ privacy and possessions. I further agree to be in my area with lights out at the designated time.

3. Keep my cell phone off and in the sleeping quarters. I will not use my cell phone at any time during the week unless given permission from staff.

4. Abide by the “off limits” rules for my own safety, remaining with the group unless otherwise advised.

5. Not bring, carry or use alcohol, drugs, or weapons of any kind.

6. Refrain from romances and sexual liaisons. Maintain a brother-sister/sister-sister/brother-brother behavior style when interacting with the staff/other students of Teen Experience for the duration of the week and for 30 days after the class ends.

7. To support #6, I will dress in an appropriate fashion that supports the family mentality expected at Teen Experience. I will refrain from wearing revealing clothing – no short shorts/skirts, overly tight tops or bottoms, cutoff tops, tube tops, etc. Staff reserve the right to have me change into more appropriate clothing if what I’m wearing is deemed inappropriate which will not be up for debate.

8. Cooperate fully and respectfully with all staff members and instructors.

9. Set a positive example to others by abiding by the agreements and exhibiting good judgment and behavior always.

10. Be flexible.

Due to Texas State Law, Smoking will not be allowed for any persons under the age of 21. Teens will not be permitted to bring cigarettes or tobacco products to Teen Experience.

Should I not decide to abide by the above agreements, I understand that my parent(s)/guardian will be notified to make appropriate arrangements, and I will be escorted to IAH Airport for my return home.

I hereby consent to and authorize PSI Seminars, LLC (hereafter “PSI Seminars”), its affiliates, agents, representatives and all persons or corporations acting with its permission or upon its authority (collectively the “Producer”), to use my name, voice, photograph, video image and likeness, together with my quotes, letters, endorsements and testimonials, whether written or oral, in whole or in part (described as the “Communication”), in connection with the exhibition, merchandising, advertising, endorsement, promotion, distribution, sale and any other activities (the “Program”) relating to the marketing and sale of training programs, intellectual properties, or software (the “Product”) referred to in my Communication.

This right shall include the right to Incorporate and use the Communication in any video, audio, photograph, catalog, packaging, and package inserts and all other media, and to reproduce, exhibit, broadcast, transmit and distribute advertising containing the Communication relating to the Product.

I hereby assign and agree to assign in perpetuity to PSI Seminars any and all worldwide rights, including the copyrights and renewals thereof, in, and to the advertising and the uses to which it may be applied. I agree to execute in the future any further documents that may be necessary to perfect PSI Seminars’ ownership rights in any advertising or other works containing the Communication.

I certify that I am a bona fide user of the Product.

I certify that my endorsement of, or testimonial about, the Product in the Communication is true and accurate and reflects my honest beliefs and real experiences in the quality and benefits of the Product that can be experienced by an average consumer. If at any time, I believe that my endorsement or testimonial as contained in the Communication is no longer true, accurate, and reflective of my experience with the Product, I will promptly notify the Producer at the address below.

I am providing the Communication and my endorsement or testimonial of my own free will, and I have not been paid or compensated in any way for giving my endorsement or testimonial.

I acknowledge that the Producer has relied on my statements in producing the Program and selling the Product.

I waive any and all rights to inspect or approve the Communication, advertising, or the uses to which it may be applied as provided for herein.

The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact.

PSI WORLD, AND PSI SEMINARS, LLC strongly recommends vaccination prior to attending any seminars and/or indoor gathering.

PSI WORLD, AND PSI SEMINARS, LLC has put in place preventative measures to reduce the spread of COVID-19; however, PSI World and PSI Seminars cannot guarantee that you will not become infected with COVID-19. Further, attending our seminar could increase your risk of contracting COVID-19.

By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I may be exposed to or infected by COVID-19 by attending a PSI Seminar’s event and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at the seminar may result from the actions, omissions, or negligence of myself and others, including, but not limited to, PSI employees or staff, volunteers, and program participants and their families.

I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I may experience or incur in connection with my attendance at, or participation in PSI Seminars (‘Claims”). On my behalf, I hereby release, covenant not to sue, discharge, and hold harmless PSI, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of PSI World or PSI Seminars, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any PSI Seminars program.
